How to Apply for Your Pension Benefit

 

If you are retiring or leaving your workplace, you should file an application for your pension benefit.

You can easily request this application through the "My Messages" section on your "My Local 338" account, or by clicking here. Once you have filled out the form, you can also submit it via message!

Different pension rules determine which pension benefit you are eligible for and what your pension benefit will be. Please consult your Plan booklet - the "Local 338 Retirement Fund Summary Plan Description" for more information. This is also available on your Member Portal under "Forms and Documents."

If you have any questions about your Pension Benefits, feel free to contact the Pension Department at (516) 294-1338 extension 1305. Forms can also be mailed to the Local 338 offices at 1505 Kellum Place Mineola, New York 11501 if necessary. However, your Member Portal will allow for timely, efficient, and secure submissions of your application. View and request any forms you may need through your "My Local 338" account under "Forms and Documents" or by clicking here.
